This privacy policy, as part of the consent form, sets out how Dietitian Ellen uses and protects any personal information that you can provide us.
Dietitian Ellen is committed to ensuring that your privacy is protected. Should we ask you to provide certain information by which you can be identified when using our services, you can be assured that it will only be used in accordance with this privacy policy.

We collect your personal data because you give us consent to do so, in order to provide independent nutrition and dietetic services to you.

We may collect information in relation to this consent form and further clinical information in relation to reports from other health professionals.
What we do with the information we gather:
We require this information for the purpose of professional clinical record keeping of client information and sharing information with relevant health professionals.
The information collected is used to help formulate your dietetic plan. Data will not be used for any purpose other than for which it has been collected.
Security
We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place encrypted electronic systems and standard operating procedures to safeguard and secure the information we collect.
Controlling your personal information:
We will not distribute, sell, or lease your personal information to third parties unless we have your explicit permission or are required by law to do so.
You may request details of personal information which we hold about you under the General Data Protection Regulation (GDPR). If you would like a copy of the information held on you, please contact dietitianellen1@gmail.com requesting this.
If you believe that any information we are holding on you is incorrect or incomplete, please email dietitianellen1@gmail.com as soon as possible. We will promptly correct any information found to be incorrect.
How you can withdraw and request to be deleted from our files:
If you do not wish us to make use of your personal information in this way, please email dietitianellen1@gmail.com You have the right to withdraw consent. Please note if you do withdraw your personal information, a new initial consultation will need to be booked prior to any further review consultations.

Social Media
We are a user of Instagram, TikTok and Facebook and interact with other users by creating a public profile which shares photos, videos, and written content. It is also used to share links from the Dietitian Ellen website and may be used to share content which has been produced by third parties (such as sharing relevant news articles).
If you comment on the Dietitian Ellen Instagram, TikTok or Facebook page or send a private message, we will assume you give permission for us to respond. We have no control or responsibility over the processing of your personal data by social media providers or any other social media account.
